Fall 2026 Applications will be OPEN TILL Monday, October 12th at 11:59 PM for DPH-2 and DPH-3 students!
The application consists of three essay questions linked below:

Please send your completed application in PDF format with the title PLS application-last name, first name to Madison Paal (mpaal@wisc.edu) no later than Monday, October 12th, 2026 at 11:59 PM.

Upon acceptance to PLS, each member will be required to pay a one-time fee of $140 to the National Phi Lambda Sigma organization.
