Registration Policy
Students must be properly registered for Pharmacy Practice Experience courses by the first day of class. Valid registration is defined as: 1) Approval via pre-registration through the Experiential Learning Program Office; 2) Course registration via the UW-Madison enrollment system; 3) Completion of all School of Pharmacy health and BLS requirements; and 4) Completion of a Background Information Disclosure (BID) form; 5) Completion of HIPAA training.
Registration must be complete to adhere to University policy for student liability insurance coverage. Students who report to a Pharmacy Practice Experience site without a valid course registration will be instructed to leave the site immediately. The student will not be permitted to register retroactively for this Pharmacy Practice Experience. The student may enroll in a Pharmacy Practice Experience in a later block or module if a site is available. This, of course, could have several undesirable consequences, including delay of graduation.
As noted above, valid registration in a Pharmacy Practice Experience course also requires completion of all health policy requirements. Prior to pre-registration each semester Student Services will verify student’s health status. Any student who has not completed health and BLS requirements will be dropped from the Pharmacy Practice Experience course and the student will no longer hold a valid course registration.
All students admitted to the School of Pharmacy are required to complete a background check and Background Information Disclosure form prior to their enrollment. Any BID report which may affect Pharmacy Practice Experience placement will be dealt with on a case-by-case basis by The Caregiver Agency, Experimental Learning Program Office, and the Associate Dean for Academic Affairs.
For International Students
Before an international student with an F-1 visa may participate in an internship outside the School of Pharmacy, s/he must contact Mara Kieser, Assistant Dean of Experiential Education, Room 1020 Rennebohm Hall, 262-6168, to obtain a letter with clerkship information and an Application for Curricular Practical Training for Pharmacy Students. This information must be submitted to the Office of International Student Services (ISS), Room 217 Red Gym, 716 Langdon St., 262-2044. Questions regarding curricular practical training regulations may be directed to ISS. If you are a J-1 student, the process is similar. Please contact ISS for information.
