The Family Educational Rights and Privacy Act of 1974 (FERPA), as amended, requires that you be advised of your rights concerning your education records and of certain categories of public information which the University has designated “directory information.” It is UW–Madison policy to comply fully and fairly with the Act. Basic university policy concerning compliance with the Act is contained in a document adopted April 1999 and titled UW–Madison Policy on Student Records. You may obtain a copy of the policy from the:

In the School of Pharmacy, if a student wishes to inspect and review his/her education records, a written request must be submitted to the student’s academic advisor. An appointment will be made as soon as possible for the student to inspect and review the file, but not later than 45 days after the student’s written request. The student’s identification will be verified. A staff member will be present while the student inspects the file.
